How to Register to participate in a Sport?
Step 1. Online Registration
In order to participate in athletics at Roosevelt High School, each family must complete the Online Registration process. During Online Registration, families should select ALL sports of interest, complete all required fields, pay all participation fees and upload the current school year physical for review by the Activities office.
Athletic Fees
A $15 athletic fee is assessed for EACH sport a student participates. Payments may be made through the parent portal or by cash/check payment directly to the Activities office. Please click on this link to access Infinite Campus
Please Note: Several sports and activities have additional individual expenses throughout the season, check with your coach for specific information.
Physical Forms: Physical forms should be uploaded during online registration. If, you are unable to upload a physical online, you may email it to Kathleen.Schmitt@dmschools.org or a hard copy may be submitted to the Activities office. Please click on the link above to access the electronic form to download and take to your physician. After September 17, you may use the below Athletic Passport link to upload your scanned documents.
All students who participate in athletics are required by Iowa Law to have a current athletic physical on file with the school. Athletic physicals are valid for 13 months from the date of the original examination.
Athletics Passport
Fill out the athletic passport online by clicking the “How to Register” link under the High School tab at the top of the DMPS Activities webpage https://activities.dmschools.org/ This link is for late registrants ONLY. Students who have completed the online registration process do not need to complete this form.
Code of Conduct and Concussion : These forms must be completed and submitted online, either during online registration, or by using Athletic Passport. Step 2. Contact the Coach
Connect with your prospective coach to express interest in a sport and for information regarding any pre-season camps, clinics, open gyms or workouts Coaches Contact Information.. Get to know all your TRHS Coaches by scanning their QR Codes on the Athletics and Activities Wall in the South Commons
Step 3. Preseason Eligibility Check
Before the start of your season, stop by the Activities Office and make sure you have a current physical on file, your participation fee is paid and all required documents have been signed and uploaded to Infinite Campus. Once all steps have been completed, you will be issued your Roosevelt BLUE Card. This card is to be presented to your coach at first practice to verify your eligibility status.
