Athletic Participation Information

In order to participate in athletics at Roosevelt High School, each family must complete several forms and submit online to the Activities office.

A $10 athletic fee is assessed for each sport a student participates in, except boys and girls golf which is $30. Several sports and activities have additional individual expenses throughout the season, check with your coach for specific information. Payments may be made through RevTrak on the DMPS web site or cash/check payments may be made directly to the Activities office. Please click on this link to access RevTrak.

Physical Forms: Physical forms may be uploaded during online registration, emailed to kristin.cooper@dmschools.org, or a hard copy may be submitted to the Activities office. Please click on the link below to access the electronic form to download and take to your physician. After September 17, you may use the below Athletic Passport link to upload your scanned documents.

Code of Conduct and Concussion forms: These forms must be completed and submitted online, either during online registration, or by using Athletic Passport (link below) after September 17.

 

Athletic Fees
You can now pay your athletic fees online!! Just click on the link and log in to your school account.

Athletic Physical Exam Form
Download the physical examination form to take to your doctor once you’ve scheduled a sports physical. All students who participate in athletics are required by Iowa Law to have a current athletic physical on file with the school. Athletic physicals are valid for 13 months from the date of the original examination.

Emergency Information Form – completed for each sport
Download the athletic emergency information form, fill it out, and turn it into the activities office. These forms are filed with our coaches in case of emergency during practice or competition.

Athletics Passport
Fill out the athletic passport online. Student must be present to co-sign several of the forms.
