Health Information from the Roosevelt Nurse!

There are two of us in the Roosevelt Health Office—Hailey Risius (Health Associate) and Nicole Cable (School Nurse).  We’re available to help 8am-3:45pm.  We can be reached by phone 242-7274, email ( nurse or health associate), or message us through SchoolCNXT (nurse or health associate).  You can also book a virtual appointment with us using this link.

After this initial message, we will transition to using only SchoolCNXT for our health office messages.  This messaging app has lots of advantages–it works for 100 languages and can even read messages aloud for you.  If you have not yet signed up for SchoolCNXT, now is a great time to do so–please email Melissa Floyd for assistance.  If you have accessed SchoolCNXT in the past but forgotten your password, click on the forgot your password link.

We can answer questions about illness, injuries, immunizations, community resources, and COVID-19.  If your student tests positive for COVID-19 (needs to isolate) or is identified as a close contact (needs to quarantine), please let us know.


This year it is more important than ever to get a flu shot! 
Take the whole family and get one today.  You can get a flu shot from: your doctor, Polk County Health Department, or many local pharmacies (Hy-Vee, Target, Walgreens, CVS, etc.).

Remember, free meals are available to all students age 18 and younger this fall, regardless of income.  More information can be found here.

For those of you planning to come back to school in the hybrid model starting November 10th, things will look at little different.

  • All students and staff will need to self-monitor for symptoms of illness BEFORE coming to school.  STAY HOME if there are any symptoms of illness.
  • Face coverings or masks will be required for everyone throughout the school day.  More information about what counts and what does not count as a good face covering can be found  here on the CDC website.
  • Maintain physical distance of 6 feet from others as much as possible (in classrooms, lunch room, hallways, etc.).
